Informed Consent for Online EQ Testing for Purposes of Personnel Development
Healthcare demands a certain level of understanding for patients who might be experiencing pain, fear, anxiety and grief. To ensure that this is given to the patients, clients, and other stakeholders, healthcare workers will need to be equipped with emotional intelligence. Their emotional intelligence will allow them to provide compassionate, patient-centered and effective care to patients as well develop coping skills with life stressors. The current test will measure a healthcare worker’s emotional intelligence.

Terms and Conditions
I. Goals and Scope of Psychological Testing
Personality Testing refers to the assessment of a test-taker’s character or trait, or in this circumstance, level of emotional intelligence. The goal of this testing is to establish a baseline/improved EQ profile of all regular employees in the hospital. The purpose of which is to help the agency create/enhance programs to better the healthcare workers’ response to patients and other clientele as well better the healthcare worker’s own coping with life stressors.
II. Anchor for the Testing
This testing has been approved by the Medical Center Chief II of the hospital. The approval is indicated in MCC Memorandum No. 03-009 s of 2026.
III. Privacy and Confidentiality
Normally, psychological results are considered to be private and shall be accessible only to the client who has taken the test. However, since the purpose of this testing is for employee development, access to the result may be needed by those who are in authority over the test-taker. Those who are in authority would be the BCSS committee, the HR Head and the MCC. If on the other hand the test-taker allows his or her head to access his or her result, no restriction will be imposed by the agency.
